3) Select a Plan and Click ‘Enroll’

4) Click ‘Purchase Here’

5) Create Your Account

6) Broker Name Drop Down Box: Select David Glasker or Jacob Glasker

7) Fill out the Applicant Information

8) Fill out Billing and Payment Information

Note: Only Credit or Debit Card Used for Payment (Visa, Mastercard and Discover accepted).

9) Account Created and ID Card should be available
